Alopecia Areata Treatments

Types of alopecia areata include the following:

• Patchy alopecia areata: Patchy alopecia areata, as the name suggests, is characterized by patches of hair loss.
• Alopecia totalis: Alopecia totalis is a condition that a severe alopecia areata may progress into, which, from patchy hair loss becomes complete baldness.
• Alopecia universalis: Alopecia universalis is a condition that describes an overall loss of hair, not just of the scalp hair but other body hair as well like the eyebrows, eyelashes, and pubic hair.
• Ophiasis Pattern: Ophiasis is a type of alopecia areata characterized by a band-like shape of hair loss.
• Sisaipho Pattern: Sisaipho is characterized by extensive hair loss but excluding that around the periphery of the scalp.

Alopecia areata is an autoimmune condition, meaning the body’s immune system mistakenly attacks the hair follicles, leading to hair loss. While the exact cause isn’t fully understood, several factors may play a role:

• Genetics: It can run in families
• Immune system changes: Loss of the hair follicle’s normal immune protection
• Triggers: Stress, illness, or other environmental factors may contribute in some cases

Alopecia areata is usually diagnosed based on how the hair loss looks and the pattern of shedding. In most cases, no testing is needed.

Clinical evaluation:
Your doctor can often diagnose alopecia areata by examining the scalp and other hair-bearing areas. It typically causes smooth, round patches of hair loss.
In some cases, changes in the nails—such as small pits or ridges—may also be seen, especially in more extensive disease.

Additional testing (if needed):
If the diagnosis is not clear, a few simple tests may be used:
Dermoscopy: A handheld magnifier used to closely examine the scalp. It can show characteristic patterns that help confirm the diagnosis and rule out other causes of hair loss.

Skin biopsy (rare): A small sample of skin may be taken to confirm the diagnosis or rule out other types of hair loss, such as scarring alopecia.

Assessing severity
If needed, your doctor may use a scoring system (called the SALT score) to estimate how much of the scalp is affected. This helps track progression and response to treatment.

There is currently no cure for alopecia areata. Treatment focuses on reducing inflammation and supporting hair regrowth. Some patients improve on their own, while others benefit from treatment. Depending on severity, your doctor may recommend the following:

Medical Treatments:

• Corticosteroids: These are commonly used first-line treatments. They can be applied to the skin, injected into affected areas, or taken by mouth in more extensive cases.
• Minoxidil: A topical treatment that may help stimulate hair regrowth.
• Other immune-modulating therapies: In more stubborn or widespread cases, additional medications may be considered.

Emerging treatments
Newer targeted therapies (such as JAK inhibitors) are being studied and are becoming an option for some patients with more severe disease.

Other supportive options:

• Light or laser-based therapies: May help in select cases, though results can vary.
• Psychological support: Hair loss can be emotionally difficult. Support groups and counseling can be helpful, especially when hair loss is more extensive or persistent.
